Scott Bolhack, MD, MBA, CMD, CWS, FAAP, FACP, has been in practice in Tucson since 1992. He began his private practice in Catalina, Arizona, and maintains an outpatient practice in Oro Valley in the Northwest Medical Center at Rancho Vistoso. Presently, he holds the following positions: chief executive officer (CEO) of the Tucson Long-Term Care Medical Group, Inc; CEO of TLC Pain & Palliative Medicine Practice, Inc; president and CEO of TLC Medical Consulting Group, LLC; and president and CEO of TLC HealthCare™ Quality Improvement Organization, Inc. He is a clinical assistant professor with the Department of Medicine at the University of Arizona. Tucson Long Term Care Medical Group, Inc. was awarded the winner in the Business Growth Category 1-19 employees 6th Annual Wells Fargo Copper Cactus Awards in 2003; TLC HealthCare Companies now has over 55 employees.
Dr. Bolhack speaks nationally on the subject of rehabilitation in nursing homes and has gained extensive experience in the area of geriatrics. Dr. Bolhack has additional experience as a medical director for nursing homes, assisted living facilities, hospices, and home health agencies. He is a fellow of the American College of Physicians and a member of numerous professional societies, including the American Medical Directors Association, American College of Physician Executives, American Academy of Home Care Physicians, and American Academy of Hospice and Palliative Medicine.
He has presented over 20 scientific posters in the areas of quality improvement, pressure ulcers, and subacute care in skilled nursing facilities.
Dr. Bolhack earned his bachelor of science in biology from Bucknell University, his doctor of medicine from the Health Sciences Center of Brooklyn, and his masters in business administration from the Eller College at the University of Arizona. He is board certified in internal medicine, hospice and palliative medicine, and pediatrics. He has additional credentials as a certified wound specialist.
Shirley Grant, LPN
Administrative and Research Assistant
Shirley has many years of nursing experience in a variety of health care settings. She joined the TLC team in 2002 and has been actively involved in developing tools to enhance the quality assurance / quality improvement programs and processes for our contracted facilities.
Shirley has worked with the Resident Assessment Instrument (RAI) / Minimum Data Set (MDS) process and served as an assistant director of nursing in a long-term care facility. She has a special interest in comprehensive assessment issues and how these relate to Quality Indicators, Quality Measures and most importantly to the quality of care received by the residents. Through sharing of this information with our partners, she enhances the understanding and the usefulness of this “snapshot in time”.
Barbara joined TLC in September 2004. The major focus of her nursing career has been in the care of the elderly. She brings with her a wide range of interests, including regulatory issues, quality assurance, MDS Coordination, and over 5 years experience as a nursing home Assistant Director of Nursing and Case Manager. This background gives her a unique understanding of the importance of quality care in healthcare settings. Barbara is also wound care certified and is available as a wound consultant to our providers and to our affiliates in the community.
Rebecca Torres
QIO Program Coordinator
Rebecca joined TLC in February 2007. Rebecca has an extensive office background which includes over 5 years of business office experience working with corporate, non-profit and medical agencies. She is working closely with the QIO team to deliver services to numerous clients in Tucson, Green Valley, and Phoenix. She also focuses on the administrative responsibilities and is working closely with the proprietary database for reporting.
Salvatore Volpe, MD, FAAP, FACP, CHCQM

Dr. Volpe has been in primary care practice since 1990. He is board certified in Pediatrics, Internal medicine, Geriatrics and Quality Assurance. He has served as a medical director or associate medical director for several national and regional Managed Care Organizations. He is member of the Board of Directors of a Medical Liability Mutual Insurance Company and member of Medical Advisory Boards for several Managed Care Organizations.
Dr. Volpe has been interviewed by such diverse media sources as Crain’s NY Business, Medical Economics Magazine and Time Warner NY 1. He currently serves on the medical editorial board of Medical Economics Magazine and travels around the United States lecturing on topics involving Health Information Technology, such as e-Prescribing and Electronic Health Records.
Dr. Volpe is the recipient of the 2007 IPRO Quality Award. The award was presented in recognition of his commitment to improving care for Medicare beneficiaries, as reflected in the work he has done to create a patient-focused environment within his practice, and his work as a physician champion in promoting the use of electronic health records and health information technology for quality improvement.
